Diagnosis
Most faults are in the transition at the top of the backswing

is the club being swung much too hard with the big muscles (shoulders) in order
to achieve greater distance

are the shoulders dominating the initiation of the downswing, at the expense of
the arms swinging and the hips moving laterally left

is the club being thrown over the top at the start of the downswing (see photo
